Intake Criteria

Here at Gfly Healthcare Services, we offer an array of healthcare services in various settings: hospitals, long term care facilities, clinics, corporate settings, private homes, and group homes. For private settings, our associates provide care and support to clients’ (and their families), 5 years of age and up. Our associates are highly trained professionals, guided by our values and beliefs which underline cultural respect and diversity. Our registered associates are also certified by their individual colleges. Our intake process starts with our administrators receiving a referral. A referral can come from various sources (hospital, long term care, private or corporate). Our referrals are generally received by fax, email, or phone. Our administrators gather all pertinent information (i.e.: client and/or patient demographics, service(s) they are requesting, diagnosis, medications, mobility, cognition, mental health, history, pertinent documentation, etc.). Once the information is received, the referral is passed on to a nurse. The nurse will review the documents/information and arrange an initial assessment visit. Once the initial assessment is complete the nurse will create a care plan (the nurse practitioner is consulted if required) based on the client’s history and initial assessment. The client will then be monitored going forward and will receive follow up/interim assessments every 6 months or on an as needed basis. Once the treatment plan is complete a discharge assessment and report are completed as well and placed in the clients file.
